Toshiba America Business Solutions, Inc., and its affiliates and related companies (“TOSHIBA,” “TABS,” “we,” “our,” or “us”) take our obligation to protect your information very seriously.

Please note that this Request Form may be used if you are a California resident who wishes to submit a request to know the categories or specific pieces of personal information collected, sold, or disclosed by TOSHIBA, correct any inaccurate personal information, or to delete that personal information. By submitting this Request Form, you certify that you are a California resident who wishes to exercise your rights to make a request to know or delete under the California Consumer Privacy Act (CCPA). You understand that TOSHIBA is required to keep a record of your request for at least 24 months, including any reference number assigned to your request, the request date and nature of the request, the manner in which the request was made, the date and nature of your response, the basis for any denial of the request if the request is denied in whole or in part, and copies of any signed declarations you may submit for verification purposes.

You understand that we need to be reasonably sure that you are making this request to know or delete your own information, or that you are authorized to make this request about someone else’s information. Therefore, you are providing the information below, which is accurate to the best of your knowledge, so that we can attempt to verify your request. You understand that TOSHIBA will use the verification data provided to cross-check information available in our records to the extent possible, and that we may require additional documentation if you are not making the request for your own information, and/or may deny my request if the information provided is insufficient for purposes of verification.
